Our services

Our Case Managers work hand in hand to help our high risk teens stabilize and navigate their way out of a complex foster care system and into a loving and permanent home.

1 : 10 caseloads
Limited caseloads allow our case managers to adopt a ‘whatever it takes philosophy, providing everything from sailing based interventions to advanced mental health and education support.
Sailing-based interventions
Every week our case managers take their teens and families sailing to build rapport and work through experience-based challenges to develop trust, camaraderie, and teamwork.
Caregiver support
In addition to traditional responsibilities, our case managers act as the basis of a teen’s support system, providing wraparound services to ensure that they are not falling through the cracks of the foster care system.
Parent referrals and support
In addition to traditional responsibilities, our case managers act as the basis of a teen’s support system, providing wraparound services to ensure that they are not falling through the cracks of the foster care system.

Who we serve

On average our teens have had 10 or more foster care placements, attended 5 or more schools, and are unlikely to reunify with their families.

They have been referred to SailFuture for specialty case management services because existing providers have been unable to meet their unique needs and manage their challenging behaviors.

Our teens’ complex trauma leads to challenging behaviors. Our team focuses on interpreting and decoding those behaviors to understand the underlying message.
